We are passionate about promoting and developing the empowerment of patients and health professionals.

We work on all levels of healthcare system transformation- rethinking of clinical pathways and health and care services so that they pro-actively support improving health literacy and patient activation. This leads to improved health outcomes and service utilisation. We help people to Get Well and Stay Well.

We are a virtual network and community and as a CIC  we are non-political and a not for profit network.

We are delighted to have secured the highest standard for Health and Wellbeing Coaching with our partners Help & Care – accreditation by the Personalised Care Institute (PCI) 

There are a number of ways we can support you as a person living with a long term health condition, health professional or commissioner of services or health insurer.

We have free online training programmes, lots of resources and host a series of expert webinars. We look forward to working alongside you.
